Requirements for Obtaining J-1 Student Intern Immigration Documents
Responsibilities of the Host Department Administrator

Documents That Need To Be Provided by the Intern to the Host Department Administrator:
- Home Institution Certification
- You need to provide proof of funding, which must meet Yale's required minimums. The funding could include Yale funding, personal funding, and external scholarships or fellowships. Personal proof of funding may include a bank statement showing available balance (ideally in English; if not, a translation should be provided).
- If you are providing personal funding and the bank statement is not in your name, please also provide a letter of affidavit of support from the owner of the bank statement (i.e., parents, family members, and friends).
- Passport Identification page
- Copy of CV
- If you have been in the U.S. before in J-1 status, please provide copies of your DS-2019 forms and previous J-1 visa stamps.
- Proof of health insurance where coverage start and end dates are clearly listed. Federal regulations require J-1 visa holders to have health insurance coverage for their entire stay (i.e., from arrival date until departure date, not just for the duration of their internship). Documents That Need To Be Directly Handled by the Host Department Administrator:
- OISS Intern Data Collection Form
- Copy of the “pending email” from EHS confirming VUPRCA status has been requested
- English Language Proficiency Form (If the intern is attending an undergraduate program conducted in English, this form is not needed. However, if their degree program is not in English, please complete this form and indicate whether the faculty supervisor conducted an interview in English or the student intern has taken a recognized standardized test of English as a second language [the department will choose which recognized test to use - TOEFL or IELTS - and what test score is an acceptable minimum]).
After Submitting All Documents to OISS
Upon review of the documents listed above, OISS will issue Form DS-2019 (immigration document) and Form DS-7002 (internship training plan). OISS will contact the department when the DS-2019 and DS-7002 forms are ready (7-10 business days after the submission is complete).
Once the forms are issued:
- OISS will create an electronic copy of the DS-7002 signed by OISS staff and share it with the departmental administrator. The Yale departmental administrator will then have the faculty sponsor (the faculty member listed in the training plan) sign and date it in ink, then scan it and create a completed DS-7002 in PDF format.
- Once the DS-7002 is complete with the signature of the faculty sponsor, the Yale department will then email Ozan Say and provide the completed DS-7002.
- OISS will create the signed DS-2019 and DS-7002 forms and email the J-1 Intern (cc'ing the Yale department), informing them that their documents are ready so that they can apply for their J-1 visa.
