STEM OPT Application Checklist for Mailing
Below is a list and explanation of the documents you will include in your STEM OPT application. Although the Form I-983 Training Plan is required for your OISS adviser to issue the new I-20 with STEM OPT recommendation, you will NOT send this form to USCIS with your STEM OPT application.
- Form I-765
- Form G-1145
- Two U.S. passport-style photographs
- Check or money order for $410
- Make photocopies of:
- All previous I-20s (you will receive one additional I-20 to add to your application after OISS review)
- F-1 visa and passport ID pages (including your photo, passport number, and expiration date)
- All previous EADs, including your current EAD
- Printed online I-94 record
- Proof of STEM Degree
- Proof of your STEM degree should include a copy of your Yale diploma (which does not need to be translated from Latin) accompanied by a degree verification letter or an official transcript.
- If your STEM eligibility on is based on a previous degree, you should include a copy of your diploma accompanied by an official transcript of this degree.
Mailing Your STEM OPT Application
Once you receive your new STEM OPT I-20 from your OISS adviser, you are ready to mail your application to USCIS.
- Include a copy of this new I-20 with the rest of your STEM OPT application (don't forget to sign your new I-20).
- Make a copy of all application materials for your records.
- Make sure to send your application via certified mail with a return receipt or tracking number.
- Your STEM OPT application must be received by USCIS within 60 days of the issuance of your new STEM OPT I-20 and before the end date of your post-completion OPT.
Your OPT application and all of the required documentation must be sent to the USCIS Lockbox facility. Find the correct lockbox and the relevant address here (look under “Foreign Students”, category (c)(3)(i)). Please note that the address of the lockbox facility is slightly different depending on your mailing method (via USPS or courier service).
